Possessive determiners come before nouns; possessive pronouns replace complete noun phrases. Neither takes a possessive apostrophe.
A possessive determiner must go with a noun: my phone, their bags. A possessive pronoun replaces the whole phrase: mine, theirs. Do not add an apostrophe to yours, hers, ours or theirs; its is a determiner, while it's means it is or it has.
